Responsibilities
- Conduct outreach calls to members to support care plan follow-ups, answer questions, and provide ongoing education
- Assist members in accessing healthcare and community resources to support their overall well-being
- Serve as the front-line resource for member and provider inquiries, including care plan procedures, scheduling support, and service protocols
- Support onboarding activities by sending welcome letters, educational materials, and related documentation
- Maintain and update non-clinical member records in compliance with state and regulatory standards
- Refer members to appropriate services to address Social Determinants of Health (SDOH) needs
- Collaborate closely with team members and leadership to meet scheduling and compliance targets
- Handle both outbound and inbound calls in a call center-style environment with a focus on member satisfaction
- Perform other administrative and support duties as assigned
