The Administrative Care Management Coordinator is responsible for performing vital administrative support within a high-volume call center. This role ensures a seamless post-hospital transition for members by proactively coordinating follow-up care and providing necessary resources. Serving as a primary liaison, the Coordinator resolves member, provider, and staff inquiries, schedules essential services, and meticulously maintains member records according to all state and regulatory guidelines. Schedule: Thursday-Monday 8:00 a.m. to 5:00 p.m.  (Must be located in the PST time zone for consideration)

Responsibilities

• Primarily making outbound calls to existing members, especially those recently discharged from hospital admissions, to discuss and support their non-clinical needs.
•  Providing recently discharged members with needed resources and assistance.
• Enrolling existing members into the appropriate Care Management Program for clinical needs.
• Making a minimum of 50 calls per day in a fast-paced environment. Performance will be monitored based on call volume and caseload metrics.
• Accurate and timely data entry into the internal system.
• Utilizing and navigating multiple computer systems simultaneously.
• Must be flexible to work Thursday - Monday Schedule and live in PST timezone
