We are seeking a dedicated, compassionate, and detail-oriented Aging and Disability Resource Connections (ADRC) Intake Specialist to join our team. In this role, you won't just be answering phones—you will be an advocate, a guide, and a lifesaver for families navigating the long-term care system

Responsibilities
Key Responsibilities
- Conduct Assessments: Perform telephone assessments of functionally impaired seniors and applicants to prioritize them for services.
- Manage Waitlists & Referrals: Place applicants on appropriate waiting lists (e.g., CCSP, HCBS) and refer them to local community resources.
- Collaborate with Professionals: Maintain open communication with medical professionals regarding applicant health and needs.
- Maintain Compliance: Ensure all documentation and screening follow state and federal aging service guidelines, and report suspected abuse or neglect to APS/LTCO.
- Data & Admin: Handle data entry and pull reports for evidence-based databases.
