Responsibilities
About the Role:
The Center for Urban Community Services (CUCS) seeks a Case Manager for The Lenninger Residence, a 92-unit permanent supportive housing community. You will provide person-centered case management to support clients in achieving housing stability, health, recovery, and self-sufficiency.
Key Responsibilities:
- Develop and update individualized service plans based on client strengths and goals.
- Assist clients with securing benefits, maintaining housing, and accessing healthcare/community resources.
- Provide crisis intervention, medication support, and assistance with activities of daily living.
- Deliver services onsite and in the field (home visits, accompanying clients to appointments).
- Maintain timely, accurate documentation and client records per regulatory standards.
- Education/Experience: Ranging from HS Diploma + 2 yrs experience (CM I) to Bachelor’s + 4 yrs experience (CM IV). College credits may substitute for experience.
- Skills: strong communication, organization, and crisis management skills; computer proficiency; bilingual preferred.
- Physical/Field Work: Ability to travel throughout NYC, escort clients via public transit, and lift up to 30 lbs. Valid driver's license may be required.
