Field Case Management Coordinator
The Field Case Management Coordinator is responsible for utilizing a collaborative process of assessment, planning, facilitation, and advocacy to support members in accessing appropriate services aligned with their benefit plan and/or health needs. This role focuses on promoting optimal, cost-effective outcomes through effective communication and coordination of available resources. Prior experience in case management is preferred.
This position requires regular travel to members' homes. Candidates must reside in or near the counties listed below to meet travel requirements. Work performed outside of member visits will be completed remotely from home. We are seeking a highly organized, efficient professional who can work independently and manage their schedule effectively.
Minimum Qualifications:
Bachelor's degree in behavioral health or human services (REQUIRED)-including psychology, social work, marriage and family therapy, nursing, counseling, or related fields.
A non-licensed master's-level clinician is also acceptable.
Residency in Illinois, within or near Boone/McHenry counties.
Interest in working with elderly individuals and persons with disabilities who receive in-home or nursing facility care.
At least 2 years of experience in behavioral health, social services, or another related field aligned with the program's focus.
Case management experience preferred.
Managed care experience preferred.
Schedule:
Monday-Friday, 8:00 a.m. to 5:00 p.m. CT
