We are looking for a RN Case Manager to join our team as a member of the Interdisciplinary Team, the Registered Nurse participates in the planning, implementation, and evaluation of care plans in accordance with established objectives, standards, and policies. This role requires proficiency in administering professional and respectful nursing care to assist frail elders in remaining safely within the community.
Essential Responsibilities:
- Direct Care & Assessment: Provide direct care to participants and complete initial and periodic assessments to develop or revise care plans based on emergent and pre-existing needs.
- Information Management: Collect medical and social data to ensure comprehensive decision-making regarding participant safety and well-being.
- Quality Assurance: Monitor clinic care and service delivery to ensure compliance with quality assurance and operational standards.
- Clinical Monitoring: Remain attentive to health status and social functioning; identify overt problems, set priorities, manage immediate referrals, and interpret diagnostic results.
- Care Coordination: Collaborate with the interdisciplinary team to develop comprehensive care plans that incorporate the perspectives of participants and their families.
- Level of Care Documentation: Participate in implementing client summaries to assist the state in determining Nursing Home Level of Care.
- Clinical Follow-through: Ensure all Physician-ordered tests, lab work, and diagnostic studies are completed.
- Education: Provide consultation and education to participants and caregivers regarding care plans, treatments, and medications.
- Communication: Maintain clear communication regarding participant conditions with primary caregivers, physicians, and the multidisciplinary team.
Job Requirements:
- Current Registered Nurse license in the Commonwealth of MA.
- Minimum of two (2) years of experience working with the geriatric population.
- Proficiency in Microsoft Office tools.
- Ability to travel locally frequently using a personal vehicle.
- Strong clinical judgment to recognize and resolve potential issues before they become acute.
- COVID-19 vaccination preferred.
