The Pharmacist will perform comprehensive clinical and operational duties, including:
Clinical Review & Documentation:
Conduct a comprehensive clinical review of pharmacotherapy for each patient, including drug utilization, counseling, compliance, drug interaction screening, side effect profiling, and drug administration support, ensuring the treatment plan aligns with evidence-based recommendations.
Document results of pharmacotherapy review in the patient chart to inform the nursing team performing follow-up assessments.
Patient Counseling & Care Coordination:
Counsel patients on new drug therapy in accordance with clinical management models.
Provide subcutaneous teaching support for patients and caregivers and, if necessary, coordinate adequate nursing services with the prescribing clinician.
Coordinate patient care with the referring clinician, addressing social, psychological, and physical needs that warrant customized services by Biologics, Inc. (e.g., pre-filled syringes, color-coded labeling) and documenting these services in their progress note.
Serve as a Liaison between patients, caregivers, home health agency nurses, hospice nurses, and the prescribing physician/clinician for orders.
Serve as primary intake for all new team referrals initiated by telephone.
Compliance & Administration:
Complete REMS training and follow program SOPs accordingly.
Implement clinical management models for products and disease states.
Provide on-call coverage at the direction of the Pharmacist-in-Charge.
Process prescriptions accurately and timely.
Verify the discontinuation of service for each patient by assigning the appropriate therapy outcome and deactivating the patient's chart, prescriptions, and software profile.
Dispense all medication pursuant to a physician's order and in accordance with all state, federal, and board of pharmacy laws.
Strictly adhere to company policy as it relates to all government funded payers, specifically Medicaid and Medicare.
Monitor hazardous waste material management within the pharmacy.
Perform intake of and ensure appropriate reporting of adverse events and product quality complaints.
Quality Assurance & Supervision:
Verify accuracy of prescription order entry.
Verify accuracy and quality assurance check of the final prescription product dispensed.
Understand and supervise the aseptic technique of pharmacy personnel.
Oversee pharmacy technicians.
Assist pharmacy technicians in adjudication of pharmacy claims.
Qualifications and Requirements
Experience: 3-5 years or more post-graduate experience in pharmacy operation, clinical oversight, and patient care.
Counseling: Must have at least 2 to 3 years of direct patient counseling experience.
Education: Graduate of an accredited school of pharmacy.
Licensure: Licensed in the state of North Carolina or Texas to practice pharmacy, license in good standing.
Certification (Preferred): BCOP certified or willing to become BCOP certified when eligible.
Skills:
Excellent oral and written communication skills.
Working knowledge of computers and excellent word processing skills.
Working knowledge of sterile technique and other quality control issues.
