Join our team at a closed-door specialty pharmacy as a Patient Care Coordinator. In this high-impact data entry role, you will be the first line of defense in the prescription intake process. You will be responsible for Rx Setup, ensuring that every referral is accurate, complete, and ready for clinical processing to get patients the medications they need.
Shift & Training Schedule
Post-Training Shift: Monday - Friday, 11:00 AM - 8:00 PM.
Training Period: Monday - Friday, 9:00 AM - 6:00 PM (On-site for approximately 2 months).
Flexibility: This role begins as 100% on-site; hybrid eligibility begins once you are fully trained and meeting performance metrics.
Key Responsibilities
Intake & Referral Review: Review referral contents to ensure all necessary clinical and demographic information is present.
Patient Profiling: Create accurate patient profiles from scanned or faxed images, entering provider and patient data with high precision.
Prescription Auditing: Conduct initial checks of prescriptions for clinical accuracy before they move through the dispensing workflow.
Provider Liaison: Communicate with healthcare providers to clarify prescriptions, request missing information, or update referral status.
Data Integrity: Update concise progress notes within the system and accurately enter NPI (National Provider Identifier) data for business reporting.
Qualifications
Required
Education: High School Diploma or equivalent.
Certification: Current CPhT (Certified Pharmacy Technician) certification in good standing.
Licensure: Must be registered with the Board of Pharmacy.
Experience: Prior experience working in a pharmacy setting.
Skills: Exceptional attention to detail, basic computer proficiency, and the ability to work in a fast-paced environment.
Preferred
2+ years of pharmacy experience.
Previous experience in a healthcare or specialty pharmacy environment.
