We are looking for Care Coordinators to join our team in an onsite role in Jenkintown, PA!
Candidate must be bilingual English/Spanish.
As a Care Coordinator Specialist, you will play a crucial role in ensuring the seamless coordination of care for our patients. You will be responsible for entering and covering shifts, and communicating with caregivers, patients, and their families. Your expertise and knowledge in care coordination will be essential in ensuring the highest level of quality care and patient satisfaction. The ideal candidate will possess excellent organizational skills, strong communication abilities, advanced care coordination abilities, and a deep commitment to providing exceptional care coordination.
ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:
1. Referral and Intake Management:
- Receive referrals from various sources, such as hospitals, physicians, and other healthcare providers.
- Review referral documentation, ensuring completeness and accuracy of information.
- Collaborate with internal teams to determine patient eligibility and initiate the intake process.
2. Care Coordination and Scheduling:
- Oversee and manage the care coordination process for a caseload of patients, ensuring continuity and quality of care.
- Create and schedule shifts, coordinating the availability of caregivers utilizing caregiver skills and client preferences. Maximize coverage based on authorized hours.
- Coordinate with patients and their families to ensure timely and appropriate delivery of care.
- Monitor and update schedules to accommodate changes, cancellations, and rescheduling needs. Ensure reasons for changes are documented.
- May be required to be part of the on-call rotation.
3. Communication and Information Management:
- Serve as the primary point of contact for patients, families, and caregivers regarding care coordination.
- Communicate pertinent patient information, including diagnosis, treatment plans, and care instructions, to the appropriate individuals.
- Maintain accurate and up-to-date electronic and physical records of patient information, appointments, and communication.
- Complete required documentation pertaining to complaints and/or incidents.
- Ensure compliance with privacy regulations and maintain strict confidentiality of patient data.
4. Collaboration and Team Support:
- Facilitate communication and information sharing among the care team members.
- Participate in care conferences and meetings to provide updates on patient status and care coordination activities.
- Assist in identifying and resolving any issues or barriers to care delivery.
- Support central and shared service teams with information related to service verification and billing.
- Support process improvement projects.
- Act as a resource and mentor to Care Coordinator Associates, providing guidance and support in complex care coordination cases.
5. Patient and Family Support:
- Provide compassionate and empathetic support to patients and their families throughout the care coordination process.
- Address inquiries, and concerns, and provide guidance on accessing services and resources.
- Educate patients and families about available personal care services, including expectations, rights, and responsibilities.
- Provide guidance and support in navigating the healthcare system, including assistance with accessing community resources and helping to coordinate additional services.
KNOWLEDGE, SKILLS AND ABILITIES:
- Proven experience in a healthcare or customer service role, with exposure to care coordination or scheduling preferred.
- Strong organizational and time management skills with the ability to prioritize tasks effectively.
- Excellent verbal and written communication skills, including active listening and empathetic communication.
- Proficiency in using electronic medical records (EMR) systems and scheduling software.
- Knowledge of medical terminology and healthcare regulations.
- Ability to work collaboratively in a team environment, demonstrating flexibility and adaptability.
- Compassionate and patient-centered approach with a commitment to delivering high-quality care.
- Strong attention to detail and accuracy in recordkeeping and documentation.
- Strong leadership and mentoring skills, with the ability to guide and support team members in complex care coordination cases.
- Strong critical thinking and problem-solving abilities to address complex patient care.
