The Field RN Case Manager utilizes a collaborative process of assessment, planning, facilitation, and advocacy to meet individual benefit plans and health needs. Through effective communication and the mobilization of available resources, you will empower members and promote optimal, cost-effective health outcomes.
This is a flexible, field-based role perfect for an organized, self-motivated Registered Nurse who thrives on working independently, making a direct impact in members' homes, and working remotely between visits.
Key Responsibilities
Comprehensive Member Assessments: Conduct thorough assessments of referred members' clinical needs and eligibility using clinical tools, data review, and direct observation during home visits.
Case Resolution & Planning: Evaluate benefit plans alongside internal and external programs/services to establish personalized, high-quality care and resolution strategies.
Guideline & Policy Application: Apply and interpret standardized case management plans, clinical criteria, policies, procedures, and regulatory standards to ensure appropriate administration of benefits.
Quality & Compliance: Execute case management and quality management processes in strict compliance with accreditation standards, regulatory guidelines, and company procedures.
Advocacy & Collaboration: Serve as a trusted advocate for members, connecting them with community resources, health education, and support systems.
Qualifications & Requirements
Licensure: Active, unrestricted Registered Nurse (RN) license in the state of Illinois (Required).
Clinical Experience: Minimum 3-5 years of clinical practical experience (Preferred).
Case Management Experience: Minimum 2-3 years of experience in Care Management, discharge planning, and/or home health care coordination (Preferred).
Location & Mobility: Must live in/near designated service areas in Illinois to support up to 50-75% travel for member home visits.

Work Environment & Schedule
Working Hours: Standard schedule is Monday through Friday, 8:00 AM - 5:00 PM CST.
Hybrid Field Model: Conduct home visits with members within your assigned region and complete administrative responsibilities, documentation, and follow-ups from your home office between visits.
Tools Provided: Necessary technology and resources to support remote work and mobile documentation.
