Position Summary: Seeking a detail-oriented and patient-focused Registered Pharmacist to oversee medication dispensing, sterile compounding, and clinical consultation. This role works closely with multidisciplinary healthcare teams to evaluate, monitor, and optimize drug therapies for home care patients.
Key Responsibilities
Medication Dispensing & Patient Education: Accept, evaluate, and dispense prescription and non-prescription medications. Counsel patients on storage, proper dosage, side effects, and drug interactions. Educate patients on medical devices (e.g., glucose meters, blood pressure monitors) and deliver clinical services like smoking cessation, cholesterol management, and blood pressure checks.
Clinical Care & Collaboration: Monitor patient medication compliance and over-usage patterns. Identify drug-related issues and collaborate directly with physicians, nurses, pharmacists, and interns to optimize patient outcomes. Evaluate drug therapies specifically tailored to home care patients.
Compounding & Sterile Preparation: Compound medications for internal and external use. Prepare sterile solutions following standard operating procedures and legal requirements.
Operations & Administration: Supervise pharmacy technicians, clerks, and support staff. Manage pharmacy inventory levels for pharmaceuticals and medical supplies. Maintain complete, accurate, and confidential patient records in compliance with pharmacy law.
Qualifications & Requirements
Education: Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) strongly preferred.
Licensure: Active Registered Pharmacist (RPh) license in good standing with the State Board of Pharmacy.
Technical Experience: Proven experience compounding infused drugs and preparing sterile solutions. Background in evaluating drug therapies for home care patients preferred.
Core Competencies: Excellent written and verbal communication skills; strong organizational and time-management abilities; strong commitment to legal compliance and patient confidentiality.
Professional Affiliations: Active membership or participation in professional pharmacy societies is desirable.
Schedule: Monday - Friday, 9:00 AM - 4:00 PM
