The role involves performing comprehensive clinical assessments to collaborate with members, their families, and internal and external partners to develop tailored, member-centric plans of care. This includes identifying and addressing barriers to meeting goals and utilizing community resources and funding as needed. Core responsibilities include ongoing monitoring and management of each member's progress, adjusting the plan and contact frequency based on their needs and acuity. The Case Manager evaluates and facilitates care across the entire continuum, from physician offices to hospitals and home care settings, and educates members to encourage proactive intervention and positive health outcomes. The role requires meticulous documentation of all aspects of the care plan, from initial assessment to final outcome evaluation. The Case Manager may also outreach directly to high-risk, high-cost, or high-utilization members and may review alternative treatment plans, assessing available benefits and the need for exceptions.
