Your daily efforts will directly support discharge planning, from actively participating in team meetings and handling paperwork to arranging patient transportation and coordinating medical equipment deliveries. You will also assist with vital financial tracking, insurance verification, and maintaining total compliance with strict HIPAA privacy regulations. This rewarding on-site position features a flexible part-time schedule centered around weekends, starting at 8:00 AM.

Responsibilities
1. Safeguards and preserves the confidentiality of patient’s protected health information in accordance with State and Federal (HIPAA) regulatory requirements, hospital and departmental policies.
2. Ensures a safe patient environment and adherence to safety practices per policy.
3. With consideration to age, employee utilizes the approved process to resolve biophysical, psychological, educational and environmental needs of patient/significant other when administering care.
Discharge Planning
4. Actively participates in daily Case Management team meetings.
5. Follows up with phone calls and necessary paperwork to ensure seamless referrals to outside agencies.
6. Collects information to facilitate assessment of financial resources available for post-acute care.
