Social Worker
Location: Southwest Ohio (Field-Based/Remote)
Pay: $27 - $29
Schedule: Monday-Friday, 8:00 AM-5:00 PM (flexibility required based on member needs)
18 Week Contract

About the Role
Leading national healthcare company is seeking a Social Worker to support children and families. This is a field-based remote position where you'll coordinate behavioral health services, connect members with community resources, and collaborate with providers and multidisciplinary teams to improve health outcomes.
This role requires up to 50% local travel to member homes and community locations. Candidates must reside in one of the eligible Southwest Ohio counties.
Responsibilities
Conduct comprehensive member assessments and identify healthcare and behavioral health needs.
Develop, implement, and monitor individualized care plans.
Coordinate services with healthcare providers, community organizations, and Child & Family Teams.
Provide education, advocacy, and ongoing support to members and caregivers.
Utilize motivational interviewing and care management techniques to improve member engagement.
Identify and address barriers to care while ensuring quality outcomes.
Maintain accurate documentation and comply with all regulatory and company requirements.
Qualifications
Required Bachelor's degree (or non-licensed Master's) in Behavioral Health, Human Services, Health Services, Public Health, or a related field (e.g., Psychology, Social Work, Counseling, Marriage & Family Therapy, Juvenile Justice).
2+ years of experience in:
Behavioral health, social services, or human services
Children's mental health, child welfare, developmental disabilities, juvenile justice, or community-based services
Case management, family systems, child development, or counseling
Knowledge of Ohio community resources and healthcare delivery systems.
Proficiency with Microsoft Office and navigating multiple computer systems.
Reliable transportation and willingness to travel up to 50%.
Ability to occasionally work outside normal business hours.
Preferred
Medicaid experience
Managed Care experience
Case management and discharge planning experience
Travel
Up to 50% local travel within your assigned territory.
Mileage reimbursement provided.
Occasional travel (1-2 times per year) to the corporate office in Franklin County/New Albany for meetings or training.
