## Position Summary
We are seeking a compassionate and experienced **Psychiatric Mental Health Nurse Practitioner (PMHNP)** to provide comprehensive behavioral health services to older adults in a variety of care settings, including participants' homes, adult day centers, assisted living facilities, and skilled nursing facilities.
In this role, you will manage a caseload of patients referred by their Primary Care Provider (PCP), conduct psychiatric evaluations, develop treatment plans, manage psychopharmacology, and collaborate with an interdisciplinary team to improve patient outcomes.
## Key Responsibilities
* Perform comprehensive psychiatric evaluations for geriatric patients across multiple care settings.
* Assess and manage acute behavioral health changes to reduce psychiatric hospitalizations.
* Provide follow-up care after psychiatric hospitalizations.
* Conduct home visits and nursing facility visits as clinically appropriate.
* Develop, implement, and monitor individualized behavioral health treatment plans.
* Prescribe, monitor, and adjust psychotropic medications while ensuring safe and evidence-based care.
* Implement Gradual Dose Reductions (GDR) in accordance with regulatory guidelines.
* Educate patients, families, caregivers, and interdisciplinary team members on behavioral health conditions and treatment plans.
* Maintain timely, accurate, and compliant clinical documentation.
* Collaborate closely with physicians, nurses, social workers, and other members of the care team.
* Participate in quality improvement initiatives and program development.
* Provide coverage for Behavioral Health colleagues as needed.
* Travel locally to participant homes and contracted facilities.
## Qualifications
### Required
* Master's of Science in Nursing (MSN)
* Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
* Active Massachusetts Nurse Practitioner license
* Massachusetts Controlled Substances Registration (MCSR)
* DEA registration with prescribing authority
* Current CPR certification
* Minimum of 5 years of experience working with a geriatric population
* Ability to travel locally throughout the service area
* Ability to pass an N95 respirator fit test (clean-shaven where required for proper mask seal)
### Preferred
* Experience working with Medicare, geriatric, or PACE populations
* Skilled Nursing Facility (SNF) experience
* Experience serving culturally diverse, underserved, or low-income populations
* Strong collaboration and communication skills within interdisciplinary teams
* COVID-19 vaccination
