We are looking for a RN Case Manager to join our team in a hybrid remote/ field based role. This role is 70% remote/ 30% travel. The RN Case Manager provides comprehensive case management for assigned members with complex needs to ensure cost effective quality of care. This professional provides quality patient care via the development, implementation & evaluation of individual patient care plans. This is a full-time position, Monday- Friday 8-4 position; no nights, weekends or holidays!
