Position Summary
As a Family Nurse Practitioner, you will work in collaboration with a dedicated clinical care team to provide evidence-based care to a specific panel of patients. Our team-based approach focuses on driving superior health outcomes through accurate assessment, diagnosis, and comprehensive management. This role requires a clinician who is self-motivated, excels in autonomous practice, and is committed to fostering active patient participation in their own wellness journey.
Key Responsibilities
Clinical Excellence
Direct Patient Care: Provide primary and family evidence-based care, including assessment, diagnosis, and treatment of acute and chronic health problems.
Preventative Health: Perform routine examinations and health screenings; counsel patients on healthy lifestyle choices, behavioral health, and wellness.
Care Coordination: Complete diagnostic tests to diagnose and treat injuries or illnesses; coordinate with other healthcare professionals to maintain comprehensive care plans.
Patient Advocacy: Encourage patients to become active participants in their well-being through education and chronic condition management.
Administrative & Operations
Clinic Management: Handle administrative duties including opening/closing the clinic, managing inventory, and maintaining a clean clinical environment.
Patient Workflow: Manage appointments, conduct patient follow-ups, verify insurance coverage, and collect payments.
Technology: Utilize information management systems and electronic communication tools to document care and coordinate with the clinical team.
Qualifications & Experience
Education: Master's Degree level Family Nurse Practitioner program.
Licensure: Current National Board Certification and valid license to practice in the State of Employment as an Advanced Practice Nurse (APN/ARNP).
Experience:
Minimum of 1 year of recent Primary Care experience as a Nurse Practitioner in family medicine or primary care preferred.
Alternatively: Successful completion of a recognized primary care training program.
Core Competencies:
Proficiency in managing chronic disease conditions and understanding quality metrics.
Ability to work autonomously without direct supervision while remaining a collaborative team member.
Willingness to obtain multi-state licensures.
Strong adaptability, organizational skills, and the ability to multi-task in a clinical care team culture.
